Information for New Patients

The first step is to call our office (203-909-6370) to make an appointment. No referral is necessary. Once your appointment has been scheduled, the next step is to fill out the online New Patient Questionnaire. If you prefer, you may download and fax or mail the PDF version of the New Patient Questionnaire instead. Please make sure that we receive the Questionnaire at least one week prior to your appointment. If you need to cancel or reschedule, please contact us as soon as possible so that your clinician can be available to other patients in need. There is a $25 charge for appointments canceled with less than 24 hours notice and a $75 charge for missed appointments without notice.

Please be aware that for all telehealth appointments (telephone or video), due to laws and regulations governing health care provision and medical licensing, you must be physically located in Connecticut at the time of the appointment.  If you will not be in Connecticut at the time of your appointment, please call us to reschedule it. 

New Patient Questionnaire (Online form)


Face-to-Face Appointments

All face-to-face appointments are conducted at our office located at 437 Orange Street in New Haven. There is convenient off-street parking in the parking lot located at the rear of our building. If you are a new patient, please bring your insurance card and photo identification, a list of medications you are currently taking, and a form of payment (credit card, check, or cash) for any amount due.  Copays and out-of-pocket payments are due at the time of each visit. 
 
Enter the building through the side entrance next to the driveway marked with our sign. Once inside the foyer, check the directory and proceed to either the first floor or second floor suite depending on your provider, and take a seat in the waiting area. Your provider will come out to the waiting area to meet you at or near your appointment time.


Telehealth Appointments (Zoom Video)

For video appointments, our practice uses the medical-grade, HIPAA-compliant version of Zoom. This version of Zoom has increased privacy and security compared to the free version. It can be used from a browser (Chrome is recommended) on a computer with microphone and camera, or from the Zoom app on any other mobile device including a smartphone or tablet. To "join the meeting" with your provider, from the Zoom app or website, simply enter the appropriate "Personal Link Name" associated with your provider. Please join the meeting a few minutes before your scheduled appointment and use your real name to identify yourself so that your provider will know who you are. Joining the meeting will bring you to your provider's virtual waiting room, and your provider will initiate the video session when ready for you. If you are having any difficulties connecting through Zoom, please call your provider or have your primary phone with you with the ringer on so that we can contact you. 

If you have a copay due, please have your credit or debit card available at the time of the appointment, or you may use the online payment processor linked below.

Our providers' Zoom Personal Link Names are listed below. You can enter your provider's Personal Link Name in the Zoom app or website to join the meeting with your provider. Clicking your provider's Personal Link Name below will launch Zoom on your device.

Make a Payment

Please click here or the button below to be directed to our secure online credit/debit card processing servicer, Cardpointe. We accept Mastercard, Visa, American Express, and Discover Card. You will be asked for your account number, which is printed on the billing statement you received from us. You may omit the account number if the patient name is the same as the billing name.  There is no additional charge when paying online but note that there is a $5 minimum payment for online payment processing. 

If you would like us to keep your credit card information on file for your convenience, you may fill out the Credit Card Authorization Form and mail, fax, or bring it to us for current and future payments.

You may also make a payment by calling our office (203-909-6370) or mailing a check or money order (Integral Health Associates, 437 Orange St, New Haven, CT 06511). Be sure to reference the patient's name or account number on checks or money orders. Please do not send cash via mail.
